Sliding-Scale Payments for Therapeutic Massage & Stretching, Lactation Care, and Infant Massage Instruction At Home
Therapeutic Massage Health Services should be affordable for all people, at all ages and stages in their life. However, the costs associated with treatment may be too much for some to bear, and without a Health Saving Account or insurance that reimburses or covers Therapeutic Massage, this may be impossible.
And, help for lactating Moms should be affordable, if the person doesn't have health insurance. (All insurance policies cover Lactation Services!) The same goes for Infant Massage Classes. (All insurance policies cover Infant Massage Classes!) These are important On-site Health Services that we offer that should not be denied to people because they are financially unable to receive them.
To that end, Mountainside On-Site Massage Therapy has partnered with The Assertive Kids Foundation™ a Registered 501(c)(3) Charity, to help provide On-Site Health Services to people who might otherwise have great difficulty with yet another bill.
The Assertive Kids Foundation™ will step in to help individuals and families dealing with affordability issues with our on-site services. Some groups Assertive Kids is willing to help include pregnant and postnatal women and kids with health issues. Eligibility will be determined on a case-by-case basis. So, even if you, or your loved one, doesn't fall into one of these categories, that doesn't mean you cannot receive help.
Payment for treatment, for those approved, will be discounted based on a sliding scale determined by assessment of proven medical and financial necessity. Please use the below form ONLY.
Data collected about financial condition will remain private. Medical information will be anonymized and may be used in various health studies relating to Therapeutic Massage or other Health Modalities. To be eligible for sliding-scale discounts based on need, you must agree to these terms.

You will be contacted back by phone, via our Care Team office, and we will let you know what is required of you to submit a claim for a sliding-scale payment. Applications are again being accepted.
What Is a Sliding-Scale payment?
After considering your unique circumstances, Assertive Kids Foundation will agree to pay for anywhere from 15% to 100% of the cost of your Therapeutic In-Home Massage. The cost of the session does not change, however, Assertive Kids will issue a promissory note to Mountainside to pay for whatever percentage of your session that the Foundation has agreed to pay, and you will only be legally responsible for paying the remainder of the cost of treatment, after Assertive Kids Foundation makes their payment contribution.
Please note that this means that any medical treatment partially or fully paid for by AK must be noted, as you cannot double-bill your insurance company for sessions Assertive Kids has already paid for. Don't do this even by accident, as it can flag your insurance account for fraud. We receive reports directly from your insurer as to what's been paid for by your insurance, if your insurer chooses to pay for any of your therapy.
